In this contribution, we present the dataset of COVID-19 reported positive cases in each of the 21 groups established by the Public Health Secretary of the State of Rio Grande do Sul (RS) between March 2020 and June 2021. We further describe how such a dataset is obtained, georeferenced, and used to support the proposed mathematical modeling, aimed to forecast, the evolution of the COVID-19 in the state. Such epidemiological data set, as well as the forecasted information of the COVID-19 from mathematical modeling are stored in a database as part of the project “Exactum - Transdisciplinary Nucleus of Theory, Modeling, Technology, and Education for Combating Outbreaks, Endemics, Epidemics, and Pandemics”.
